The Air Carrier Access Act is only protected under US law therefor only applies to domestic flights. If you are needing the letter for international flights, this is at your own risk as international airline companies are not required by law to accept the letter.
However, if you are traveling directly to or from the US, the airline must abide by US laws and accept the letter.
